Pursuant to the New York State Freedom of Information Law (1977 N.Y. Laws ch. 933), I hereby request the following records:

From the Office of Public Safety, I request copies of any material related to the granting of "Official Business" placard(s) to Senator Martin J. Golden. These include:

• Forms received from Senator Martin J. Golden or from appropriate State Senate / legislative liaison, including any application or acknowledgment letter signed by Senator Martin J. Golden;
• Forms sent to Senator Martin J. Golden regarding the granting of placard(s);
• Any correspondence related to the ongoing possession of a placard by Senator Martin J. Golden, including complaints and/or warning letters.

These forms should date from approximately April 1, 2011 or later.

The requested documents will be made available to the general public, and this request is not being made for commercial purposes.

In the event that there are fees, I would be grateful if you would inform me of the total charges in advance of fulfilling my request. I would prefer the request filled electronically, by e-mail attachment if available or CD-ROM if not.
