Pursuant to Open Public Records Act ("OPRA"), I hereby request the following records:
I request all error/accident reports (form CL-21) from the year 2015 in compliance with Chapter 8, State Administrative Code: Collection, Processing, Storage and Distribution of Blood, N.J.A.C. 8:8-5.2(c) which says (d), "errors and/or accidents that result in shipment of unsuitable blood or blood components, shall be reported to the Department within 10 days of recognition of the error."
The requested documents will be made available to the general public, and this request is not being made for commercial purposes.
In the event that there are fees, I would be grateful if you would inform me of the total charges in advance of fulfilling my request. I would prefer the request filled electronically, by e-mail attachment if available or CD-ROM if not.
Thank you in advance for your anticipated cooperation in this matter. I look forward to receiving your response to this request within 7 business days, as the statute requires.
Dear Ms. Sitrin:
This will respond on behalf of the Custodian of Records for the Office of the Commissioner (OC) in the New Jersey Department of Health (Department). No government records within the OC have been identified to me as being responsive to your request. To the extent that government records responsive to your request may exist in the Department, the same may be found in the Division Public Health and Environmental Laboratories. Accordingly, you may wish to consider filing your request in that Division. If you have any questions concerning this communication, please contact me.
