Pursuant to the Nebraska Public Records Statutes (Neb. Rev. Stat. § 84-712), I hereby request the following records:

The data record layout, data frame, or any equivalent data framework used by this agency to document maternal deaths. Materials responsive to this portion of this request should include reference to the data elements collected by this department and should not include any personal information. Please feel free to redact any personally-identifying information.

The requested documents will be made available to the general public, and this request is not being made for commercial purposes.

In the event that there are fees, I would be grateful if you would inform me of the total charges in advance of fulfilling my request. I would prefer the request filled electronically, by e-mail attachment if available or CD-ROM if not.

Greetings Adanya,

We don’t believe that we keep/have information, in the manner that it has been requested, in our normal course of business.
We are still in the development phase and do not have a database or data collection tool.
We will likely be implementing the Maternal Mortality Review Information Application (MMRIA), but are still in the early planning phase.
We are more than happy to work with you, but will need more information to proceed.
