Under the Montana Public Records Act § 2-6-1001 et seq., I am requesting copies of all license renewal application documents, submitted by David Bostwick, VET-VET-LIC 1923, between February 1, 2018 and January 24, 2020. In providing these copies, you may redact “private” information, such as social security numbers.

If these documents, or any part of the renewal application materials for this licensee were submitted electronically, please provide copies of those documents, and the dates they were submitted. This information is not being sought for commercial purposes, and the requested documents will be made available to the general public.

If your agency does not maintain these public records, please let me know who does, and include the proper custodian's name and address.

If there are any fees for searching or copying these records, please inform me of the total charges in advance of fulfilling my request. I would prefer the request be filled electronically, by e-mail attachment if available, or CD-ROM if not.

If access to the information I am requesting will take longer than a ‘reasonable’ amount of time, please contact me within ten (10) business days notifying me of when I might expect copies, or the ability to inspect the requested records.

Thank you for responding to my request for public records. Unfortunately, I have received only one document, VET-VET-REC 12174, dated October 22, 2019, that appears to be a final summary of the materials contained in the licensee’s submitted application.

To clarify, I am requesting a copy of the actual license renewal application documents, including any supplemental documentation needed to complete the renewal process, submitted by Dr. David Bostwick (with “private” information, such as social security numbers redacted). Also, any documents pertaining to Dr. Bostwick’s license renewal application for 2018, are missing.

Would you please provide all the documents requested, or cite each specific statutory exemption you feel justifies the refusal to release the information, and notify me of the appeal procedures available to me under the law. Please see my original request below.

Hi Teresa: Dr. Bostwick did not provide supplemental documentation with his renewals. He is not required to unless he is pulled in a random audit of continuing education by the board.

Thank you. I'm a bit perplexed. Of all the documents you provided, that were submitted by Dr. Bostwick, his license renewal application for 2017 is missing. Does this mean he did not go through the renewal process that year? If so, how could it be that his license was not suspended or reviewed? What documents would be available regarding failure to file a renewal application?

Upon closer examination of the files you provided, I realize I am not able to see some information. For example, the “CE statement” and “disciplinary statement,” are hidden. I realize that the renewal process can now be completed online. Are digital documents of the actual license renewal application with all fields visible, available in some form?

Hi Teresa: The online application is filled out and the information we receive is what was sent to you. The paper renewal with the wording of the discipline and CE questions can be found at the link below and is the same as the wording on the online renewal:

Thank you. Are you saying that there is no possible way to get a copy of the completed license renewal application, or even an image of the document, in it’s entirety (excluding private information such as social security numbers), unless the licensee fills out and submits a hard copy?

If that is the case, please be patient and bear with me for a moment longer. As you are viewing the online renewal application screen, are the words “disciplinary statement,” clickable, giving the option to display the actual statement, making it then possible to capture all the licensee’s completed information in a screenshot?

Hi Teresa: The images I sent you are from our database and are the only completed renewal information we have for an online renewal. We know internally what the disciplinary statement says, so we just have the short description. We cannot click on the words to get the entire statement.
