This is a request under the Missouri Sunshine Law, Chapter 610, Revised Statutes of Missouri. I hereby request the following records:
Any documents or information relating to the death of Christopher W. Byrd (date of birth: May 2, 1968) on or about November 4, 2014 at St. Luke's Northland Hospital, including any determination of the cause and manner of death.
The requester is a freelance journalist. The requested documents will be made available to the general public, and this request is not being made for commercial purposes. The information I obtain through this request will be used in the preparation of a news article relating to a topic of public concern. Accordingly, I request that all fees for locating and copying the records be waived.
In the event that there are fees, I would be grateful if you would inform me of the total charges in advance of fulfilling my request. I would prefer the request filled electronically, by e-mail attachment if available or CD-ROM if not. If portions of the requested records are closed, please segregate the closed portions and provide me with the rest of the records.

The Missouri Department of Health and Senior Services (Department) is in receipt of your request dated December 19, 2018, in which you have requested records relating to the death of Christopher W. Byrd, DOB: 05/02/1968, at St. Luke's Northland Hospital on or about November 4, 2014.
The Department cannot release any records that would identify Mr. Byrd as a patient; thus, the Department cannot respond as to whether or not an inspection was performed related to Mr. Byrd's death. However, a search for records of ANY inspections of the hospital during a specified date range could be made. Please respond as soon as possible as to whether or not you would like the Department to perform this search, so that records may be provided to you in a timely fashion.
Please note, the Department may charge $21.70 per hour for research and $.10 per page for records. An invoice may be sent to you for any charges incurred in the fulfillment of your request prior to your receipt of the requested records.
