Pursuant to the Michigan Freedom of Information Act, I hereby request the following records:

1. All forms used by MDHHS to collect data on COVID-19 cases, deaths, and outbreaks in long term care facilities in Michigan between 3/1/2020 and the date this request is ultimately processed.
2. All forms used by MDHHS to collect data on COVID-19 cases, deaths, and outbreaks in hospitals in Michigan between 3/1/2020 and the date this request is ultimately processed.

To clarify what could potentially be interpreted as confusing language, I am not asking for all of the forms that were submitted to MDHHS by these facilities. Rather, I am asking for the forms used by hospitals and LTC facilities to report COVID-19 data to MDHHS.

If further clarification is needed about the type of documents I am seeking, feel free to let me know.

The requested information will be made available to the general public, and this request is not being made for commercial purposes. Disclosure of the requested information to me is in the public interest because it is likely to contribute significantly to the public's understanding of the operations and/or activities of the government, and is not primarily in my commercial interest. I am willing to pay up to $25 for the requested information. If the fee for this request exceeds this amount, please notify me in writing for permission prior to beginning work on the request.

I would prefer the request filled electronically, by e-mail attachment if available or CD-ROM if not.

Thank you for your message. Upon logging into the portal, I see $0.00 as the balance due, and there is no invoice. I've attached screenshots to confirm there is no invoice attached to this request.

Additionally, it seems highly unlikely that there would be a fee to provide me with copies of the blank forms provided by MDHHS to healthcare providers for the purpose of data tracking, as pulling up the requested blank form (if one exists, that is) should take your staff less than a minute. Can you please clarify?

Following up on MDHHS Public Records Request, Reference # H014028-122321, there is still no invoice attached to this request in the MDHHS FOIA portal. The amount due for this request still says $0.00, and there is no invoice. See attached screenshot as evidence. The request is asking for copies of unfilled documents meant for tracking. Can you please let me know what's going on with this request? Thank you.

The Department sent you an invoice for this request on January 6, 2022 and payment was never received. Per MCL 15.234 Sec. 4(14) since the invoice was not paid within 48 days your request was abandoned. You are welcome to submit a new request on our website.
