- Cause of death report for Eugene Gates, Jr., DOB 2/25/1957, DOD 6/20/2023
The requested documents will be made available to the general public, and this request is not being made for commercial purposes.
In the event that there are fees, I would be grateful if you would inform me of the total charges in advance of fulfilling my request. I would prefer the request filled electronically, by e-mail attachment if available or CD-ROM if not.

This case is still pending at this time, the autopsy report takes approximately 60 to 90 days to finalize. You would need to request when the report is complete.

I'm following up on the following Texas Public Information Act request, copied below, and originally submitted on July 3, 2023. You had previously indicated that it would be completed on Oct. 4, 2023. I wanted to check on the status of my request, and to see if there was a new estimated completion date.

Good morning,
Eugene Gates' autopsy report is now complete. To request a copy of his autopsy report or cause of death report please mail your written request with payment to Dallas County Medical Examiner's Office 2355 N Stemmons Freeway Dallas, TX 75207 with attention to Records.
The certified autopsy report is $15.00, it has an affidavit attached to it and is a legal document. The non-certified autopsy report is $5.00. Both autopsy report types have the toxicology report included in it.
If you just want to order the cause of death report, that report is $5.00. Please send either exact cash, check, or money order with your written request. If paying by check or money order, please make it payable to Dallas County.
