Please provide records sufficient to show the number of submitted Medical Request Form Forms (https://app.smartsheet.com/b/form/ce71b30ce7ef4ed0ac5b27cababb7f6f) per week and district and type of request (Shelter Placement, Isolation Needed, Appointment needed, Medical Follow up).

If this would require the construction of a new record, and the agency is unwilling to do so, please provide an export of SmartSheet records with all but the submission date, district, and Type of Request fields redacted.

Please provide records for all form submittals. If providing all records would be unduly burdensome, please provide all submittals received from August 1, 2023 to present.

The requested documents will be made available to the general public, and this request is not being made for commercial purposes.

In the event that there are fees, I would be grateful if you would inform me of the total charges in advance of fulfilling my request. I would prefer the request filled electronically, by e-mail attachment if available or CD-ROM if not.
