AZ Diamondback Correctional Facility evaluations, after-action reports

To Whom It May Concern:

Pursuant to the state open records law, Ariz. Rev. Stat. Ann. Secs. 39-121 to 39-122 and 39-128, I hereby request the following records:

All evaluations, inspections, reports, and related materials regarding all services, equipment, and activities involving/provided by/contracted for regarding Diamondback Correctional Facility and any facilities associated with it, either before, during, or after the contract term for which the facility was under obligation to the Arizona Department of Corrections.

Responsive materials should include but not be limited to:

• all annual inspections
• all emergency event reports and evaluations
• all records of inspections conducted in accordance with DOC provisions, such as those of the American Correctional Association
• all reports regarding the transfer of prisoners
• all reports regarding the performance of the contract and associated duties

The requested documents will be made available to the general public, and this request is not being made for commercial purposes.

In the event that fees cannot be waived, I would be grateful if you would inform me of the total charges in advance of fulfilling my request. I would prefer the request filled electronically, by e-mail attachment if available or CD-ROM if not.
