The requested documents will be made available to the general public, and this request is not being made for commercial purposes.
In the event that there are fees, I would be grateful if you would inform me of the total charges in advance of fulfilling my request. I would prefer the request filled electronically, by e-mail attachment if available or CD-ROM if not.

Hi Tiffany - To make sure you don't have to go back and look for this additional referenced document, please note: that In the Workman's Compensation's C&R "Compromise and Release" for Joseph Rangel, there is a reference made under item #9, page 7 (comments box) that there was a written statement that claimed an additional written agreement would be produced because "Defendant's consent to this C&R is expressly conditioned upon applicant execution of a separate civil release resolving a pending disciplinary action and agreement between the city and applicant to sever the employment relationship with each party waiving and releasing any claim against the other".... (see copy attached) Can you make sure to locate this written release agreement referred to in this statement too?
In addition, right above this above handwritten statement and listed under "other", there is a reference to these serious dispute issues being included in the settlement; specifically notes as "LC 4850 benefits, LC 13C(a) claim, any possible claims to sanctions, penalties, <C.4650 s.a.p., or interest". Can you tell me what these disputed issues actually are?
