Pursuant to the Freedom of Information Act, I hereby request the following records:

All complaints submitted to OSHA regarding facilities maintained by or work conditions under Community First Healthcare of Illinois, Inc.; Forty Six Twenty One Corporation Incorporated dba Mado Healthcare - Uptown; Little Village Nursing and Rehabilitation Center, LLC; Concerto Renal Services LLC; and Presence Chicago Hospitals Network dba Amita Health Saint Joseph Hospital Chicago.

The OSHA Information System has been categorizing all COVID-19 enforcement and compliance activities with the specific code: N-16-COVID-19. I am also requesting all complaints and referrals received by OSHA since February 1, 2020, coded with N-16-COVID-19, regarding establishments based in the state of Illinois.

The requested documents will be made available to the general public, and this request is not being made for commercial purposes.

In the event that there are fees, I would be grateful if you would inform me of the total charges in advance of fulfilling my request. I would prefer the request filled electronically, by e-mail attachment if available or CD-ROM if not.

Thank you in advance for your anticipated cooperation in this matter. I look forward to receiving your response to this request within 20 business days, as the statute requires.

You mentioned that there might be "like" establishment names, so I included the inspection numbers associated with the cases I am interested in. For each case, I am requesting the initial complaint, OSHA's administrative notes on the complaint, and inspection records. I will accept redactions in accordance to the law. I saw in the response letter that you have identified me as a commercial requester. I want to highlight that I am a member of the news media and am requesting these records for news research purposes. As I noted in my initial request, I am not an employee of MuckRock and am merely using MuckRock's FOIA-management platform to file this request.

Pursuant to the Freedom of Information Act, I am requesting a fee waiver because the subject of my requested records concerns government operations and activities, the disclosure will likely result in public understanding of the subject, the contribution to public understanding of government operations or activities will be significant, and I have no commercial interest in the disclosure.
