Pursuant to the Freedom of Information Act, I hereby request the following records:
A copy of all records from the Official Personnel File (OPF) of Anthony John Lombardi, also known as Tony Lombardi, with the approximate year of birth 1943. Lombardi served in various positions in the Treasury (Secret Service and IRS), Department of Justice (USAO, Southern District of New York), and the Presidential Commission on Organized Crime. It is stated in a news article that Lombardi left federal service approximately 1992.
Lombardi is deceased as of August 18, 2015 (see obituary from Staten Island Advance attached). All privacy interests are moot.
If any potentially responsive records have been destroyed and/or transferred, then I request copies of the destruction or transfer slips as well as any other documentation relating to, mentioning or describing said transfer or destruction, to include but not be limited to confirmation that the NPRC has no other copies of said records.
The requested documents will be made available to the general public, and this request is not being made for commercial purposes.
In the event that there are fees, I would be grateful if you would inform me of the total charges in advance of fulfilling my request. I would prefer the request filled electronically, by e-mail attachment if available or CD-ROM if not.
Thank you in advance for your anticipated cooperation in this matter. I look forward to receiving your response to this request within 20 business days, as the statute requires.

This is a message to deal with some confusion.
I requested the record of Anthony John Lombardi, a Treasury Agent (a civilian civil service post). In error, I appeared to have sent it to the division of NPRC dealing with military records.
I first received correspondence from Mr. Webster, who stated that the request was forwarded to the Civilian Personnel Records facility at NPRC Annex; this was assigned number 2-23501732659. Then I received correspondence from Mr. Farr stating that there is another request given 2-23639739479.
Mr. Webster's course of action was correct; please keep up the search for records in Request Number 2-23501732659 (at Civilian Personnel Records/NPRC annex).
I believe Mr. Farr's response was an honest error. Please close request 2-23639739479 forthwith.
