BOP Records on FCI Lompoc Incident 2

To Whom It May Concern:

Pursuant to the Freedom of Information Act, I hereby request the following records:

- Any statements, reports, pictures, or video collected by BOP officials concerning then-federal inmate Michael Monsivais in the aftermath of a June 21, 2017 inmate disturbance at Lompoc Federal Correctional Complex.

For reference, Monsivias says investigators took pictures of injuries he sustained due to a flashbang grenade. He was subsequently placed in restricted housing for approximately 70 days. I therefore request any such pictures, video, findings by investigators, and witness statements collected by investigators regarding Monsivais between June 21, 2017 and September 21, 2017. Please note that, when some information is exempt from disclosure, the Freedom of Information Act requires that “[a]ny reasonably segregable portion of a record shall be provided to any person requesting such record after deletion of the portions which are exempt."

Attached please find a certificate of identity provided by Monsivais authorizing the release of documents to the requester, CJ Ciaramella.

The requested documents will be made available to the general public, and this request is not being made for commercial purposes.

In the event that there are fees, I would be grateful if you would inform me of the total charges in advance of fulfilling my request. I would prefer the request filled electronically, by e-mail attachment if available or CD-ROM if not.

- All records, including pictures or videos, in former federal inmate Michael Monsivais' privacy act file and health records from June 21, 2017 until his release from FCI Lompoc.
