To Whom It May Concern:

Pursuant to the Illinois Freedom of Information Act (5 ILCS 140/1 to 11), I hereby request the following records:

According to the CPIC privacy policy last accessed at https://nfcausa.org/files/DDF/CPICPrivacyPolicy.pdf on February 16th, 2017:

"A privacy committee of designees as determined by the Superintendent of Police will ensure that privacy and civil rights are protected as provided in this policy and by the
Chicago Police Department's information-gathering and collection, retention, and dissemination processes and procedures. The committee will annually review and
recommend updates to the policy to the Commander of the CPIC in response to changes in law, including the results of audits and inspections."

"The CPIC's privacy committee is guided by a trained Privacy Officer who is selected by the CPIC Commander to assist in enforcing the provisions of this policy and who, in
addition to other responsibilities, will receive reports regarding alleged errors and violations of the provisions of this policy, receives and coordinates complaint
resolution under the CPIC's redress policy, and serves as the liaison for the Information Sharing Environment, ensuring that privacy protections are implemented throughout
the process."

"The CPIC may retain information that is based on a level of suspicion that is less that "reasonable suspicion", such as tips and leads or suspicious activity report (SAR)
information, subject to the policies and procedures specified within this policy."

"The CPIC will conduct an annual audit and inspection of the information and intelligence contained in its information system(s)."

Therefore I request the following records, for the last five years:
