Is the subject listed on the request?

Id like to get a copy of exactly what the City received. Is that possible?

Hi Tracy! When requests are emailed, they include the subjects; when we mail or fax requests, the subjects are not included. Below is a sample of what we send to departments, with some information changed to prevent spam or fake documents being sent as a response.
