Secret Service - DHS Request
1 answer from Jack R-W.
Anyone ever get one of these from DHS - need to send a notarized form?
Your request constitutes a third party request. As you have failed to provide the required documentation, to include a notarized release from George Sweigert, we can neither initiate a search for responsive documents nor confirm or deny the existence of investigatory information pertaining to the person named in your request. To assist you in preparing a proper request, please visit http://www.gpo.gov/fdsys/pkg/CFR-2014-title6-vol1/xml/CFR-2014-title6-vol1-sec5-21.xml for information regarding the portion of the Department of Homeland Security FOIA regulations, Title 6 C.F.R., Chapter I, Part 5, Subpart B § 5.21, implementing certain provisions of the Privacy Act of 1974.
You need to provide a form DOJ-361 in order for a request under the Privacy Act to be processed. You don’t need anything notarized, you just need to swear under penalty of perjury that the information you are submitting is true.
