FOIA request release of information form.

Hello,

I have been instructed to return DHS Form 590 and also narrow the scope of what I am searching for. I am not sure who to fill out in the Representative Informatiomn form, and also am not clear as to if this is the correct way to proceed. Is this going to allow all of my information to be posted publicly?
