The data record layout, data frame, or any equivalent data framework used by this agency to document maternal deaths. Materials responsive to this portion of this request should include reference to the data elements collected by this department and should not include any personal information. Please feel free to redact any personally-identifying information.

The requested documents will be made available to the general public, and this request is not being made for commercial purposes.

WI maternal deaths are identified by information reported on the death certificate worksheet by the medical certifier. Below are the items collected pertaining to maternal deaths. Indication of maternal death may or may not appear in the cause of death.
