Pursuant to the Illinois Freedom of Information Act., I hereby request the following records:
Copies of all Certificate of Records Destruction forms submitted to your library by any city/town agency between January 1st, 2018 and the current date (October 24th, 2020).
The requested documents will be made available to the general public, and this request is not being made for commercial purposes.
In the event that there are fees, I would be grateful if you would inform me of the total charges in advance of fulfilling my request. I would prefer the request filled electronically, by e-mail attachment if available or CD-ROM if not.
Thank you in advance for your anticipated cooperation in this matter. I look forward to receiving your response to this request within 5 business days, as the statute requires.

Per your request for all copies of all Certificate of Records Destruction forms submitted to our library from any city
or town agency between January 1, 2018 and the current date October 24th, 2020, we do not get forms submitted
to us we do our own application and fill out and send in our Records Disposal certificates for disposition approval
to the Local Records Commission in Springfield, IL. Please let me know if the attached is what you are looking for
or if there is something else needed.
