Pursuant to the West Virginia Freedom of Information Act and the West Virginia Open Governmental Proceedings Act, I hereby request the following records:
Copy of electronics waste disposal and/or electronics waste recycling contracts for 2013, 2014, and 2015 years for all Wetzel County Schools facilities.
Copy of hard drive shredding and/or hard drive disposal contracts for 2013, 2014, and 2015 for all Wetzel County Schools facilities.
Copy of universal waste disposal, including florescent light bulbs and batteries, contracts for 2013, 2014, and 2015 for all Wetzel County Schools facilities.
Copy of all procurement records of light bulbs, including Bills of Lading, Shipping Documents and Invoices for 2013, 2014, and 2015 for all Wetzel County Schools facilities.
Copy of all procurement records of batteries, including Bills of Lading, Shipping Documents and Invoices for 2013, 2014, and 2015 for all Wetzel County Schools facilities.
Copy of all recycling/disposal records of light bulbs, including Bills of Lading, Shipping Documents and Invoices for 2013, 2014, and 2015 for all Wetzel County Schools facilities.
Copy of all recycling/disposal records of batteries, including Bills of Lading, Shipping Documents and Invoices for 2013, 2014, and 2015 for all Wetzel County Schools facilities.
Records of electronics waste, including computers, electronics and TVs, recycled for 2013, 2014, and 2015 including destination of material for all Wetzel County Schools facilities.
Copy of Wetzel County Schools due diligence policy (or equivalent) for handling end of life electronics. Including criteria for selecting an electronics recycler for all Wetzel County Schools facilities.
Copy of records of any fines, penalties or warnings received by Wetzel County Schools in regards to improper disposal of Universal Waste including light bulbs, batteries and electronics.
The requested documents will be made available to the general public, and this request is not being made for commercial purposes.
In the event that fees cannot be waived, I would be grateful if you would inform me of the total charges in advance of fulfilling my request. I would prefer the request filled electronically, by e-mail attachment if available or CD-ROM if not.
Thank you in advance for your anticipated cooperation in this matter. I look forward to receiving your response to this request within 5 business days, as the statute requires.

I am responding to the attached FOIA request that was faxed to our small county school system. Unfortunately, the records you are requesting are not accessible in electronic format and will take some time to obtain. You are welcome to visit our board office during normal business hours to make paper copies according to our fee schedule below:
Black and White Copy: $0.25 per page
Color Copy: $0.50 per page
CD or DVD: $1.00 per disc
