Pursuant to the Washington Public Records Act, I hereby request the following records:
A copy of the document titled "Medicaid’s PDL: fast facts for healthcare providers" that was published in April 2007. This was previously published on the HCA website at this address: http://www.hca.wa.gov/medicaid/News/documents/fact_007010pdlandpreauthfax0407update.pdf. However, the page is no longer available.
There may be multiple versions of this document from different years. If that is the case, please provide all available versions published between January 1, 2004 and December 31, 2013.
The requested documents will be made available to the general public, and this request is not being made for commercial purposes.
In the event that there are fees, I would be grateful if you would inform me of the total charges in advance of fulfilling my request. I would prefer the request filled electronically, by e-mail attachment if available or CD-ROM if not.
