Pursuant to the Washington Public Records Act, I hereby request the following records:

1. any and all Auburn School District records referencing the ATTACHED document with the label FEDERAL SURVEY and FedForm:092916 at the bottom, hereinafter referred to as FEDERAL SURVEY for the years 2015-2020.

2. any policies, guidelines, manuals and/or instructions on Auburn School District use and distribution of FEDERAL SURVEY.

3. any Auburn School District and/or Auburn School Board internal communications, documents, and emails regarding the FEDERAL SURVEY for the years 2015-2020.

4. a complete copy of any Public Records Act requests to Auburn School District regarding the FEDERAL SURVEY for the years 2015-2020 and a complete copy of all communications and documents produced in response to any Public Records Act request by Auburn School District.

5. a copy of all FEDERAL SURVEY forms distributed by Auburn School District by for the years 2015-2020.

6. a copy of all FEDERAL SURVEY forms received by Auburn School District by for the years 2015-2020.

The requested documents will be made available to the general public, and this request is not being made for commercial purposes.

In the event that there are fees, I would be grateful if you would inform me of the total charges in advance of fulfilling my request. I would prefer the request filled electronically, by e-mail attachment if available or CD-ROM if not.
