To Whom It May Concern:
Pursuant to the Washington Public Records Act, I hereby request the following records:
The vaccination status of all those declared by the Washington State Department of Health to have died from flu, influenza or influenza-related illness in the 2017-18 season. "Season" includes "flu season," winter season, or any time period the WADOH uses to measure health outcomes, including those from recommended medical interventions.
“Vaccination status” data includes deaths of those known to have been vaccinated against flu or influenza, those known to have been unvaccinated, and the numbers of those whose vaccination status is unknown.
This data is understood to be kept by the WADOH in the following categories:
• age group: 0-25
25-65
65 and older
• those known to have had pre-existing conditions
This PRR requests the data aggregated in those categories.
The requested documents will be made available to the general public, and this request is not being made for commercial purposes.
In the event that there are fees, I would be grateful if you would inform me of the total charges in advance of fulfilling my request. I would prefer the request filled electronically, by e-mail attachment if available or CD-ROM if not.
Thank you in advance for your anticipated cooperation in this matter. I look forward to receiving your response to this request within 5 business days, as the statute requires.

WAC 246-08-990<http://app.leg.wa.gov/WAC/default.aspx?cite=246-08-990> (RCW 42.56.120) authorizes the Department of Health to collect fees for providing copies either electronically or in paper format. There is no charge for inspection. After the location, nature and number of records has been determined, we will give you a cost estimate for production of the records.
