To Whom It May Concern:
Pursuant to the District of Columbia Freedom of Information Act of 1974, D.C. Code Ann. § 2-531 et seq. ("D.C. Act"), I hereby request the following records:
All procedures and associated forms (blank) relevant to the reporting of inmate injuries and death at both public and privately-run correctional facilities, should differences apply.
I also request that, if appropriate, fees be waived as I believe this request is in the public interest. The requested documents will be made available to the general public free of charge as part of the public information service at MuckRock.com, processed by a representative of the news media/press and is made in the process of news gathering and not for commercial usage.
In the event that fees cannot be waived, I would be grateful if you would inform me of the total charges in advance of fulfilling my request. I would prefer the request filled electronically, by e-mail attachment if available or CD-ROM if not.
Thank you in advance for your anticipated cooperation in this matter. I look forward to receiving your response to this request within 15 business days, as the statute requires.
Attached is agency’s response to your below FOIA request.

Hello Mr. Obebe,
Thank you very much for your help!
I noticed that your letter was addressed to another person using a different DEPT MR number, and I'd like to clarify that requests sent through MuckRock are assigned unique addresses/email addresses and may be sent by different users.
I really appreciate all of your help on all of my requests.
Thanks for the clarification, and have a great day!
And you as well, sir!
