Pursuant to the Virginia Freedom of Information Act, I hereby request the following records:

I am requesting an opportunity to inspect or obtain copies of public records that include an updated list of all Virginia State Medicaid healthcare providers with the inclusion of their National Provider Identifier (NPI) number. I am also requesting ALL Virginia state licensed healthcare providers in electronic data set with the inclusion of their state license number and, if available, their National Provider Identifier (NPI) number (CSV electronic data format is preferred; however, excel or PDF electronic formats are also acceptable). The Medicaid and state health care licensee list may include but is not limited to Physicians (MD, DO), Physician Assistants, Registered Nurses, Clinical Nurse Specialist, CRNA, Nurse Practitioners, Midwives, Dentists, Podiatrists, Dental Assistants, Chiropractors, Physical Therapists, Optometrists, Occupational Therapists, Social Workers, Marriage and Family therapists, Psychologists, Emergency Medical Personnel (EMTs, Paramedics), Respiratory Therapists, Radiology Technologists and any other available providers. All types of healthcare practitioners from all dates electronically available would be appreciated. Specific items to be included in data set:

1. Provider name
2. National Provider Identifier (NPI) number
3. Virginia State Healthcare License number and initial date of issuance or expiry (if available/applicable)
4. Education / School/Training, if available
5. Provider type (MD, DO, PA, RN, PT, OT, OD, CRNA, DMD, DDS, DPM etc)
6. Provider Specialty
7. Provider Practice Location
8. Please include Data Dictionary describing state Medicaid and state Licensure Data Sets
9. Please provide (if there is an existing) a static website link to verify an individual’s state healthcare licensure status provided by any and all available state healthcare licensing boards (Medical Board, Nursing, Dental, Optometry, Podiatry, Psychology, PT, OT, Radiologic Technologist, Respiratory Therapy, Chiropractor, Speech Therapy, Family Counseling, Social Work, etc).
If there are any fees for searching or copying these records, please email me if the cost will exceed $50. However, I would also like to request a waiver of all fees in that the disclosure of the requested information is in the public interest and will contribute significantly to the public’s healthcare transparency, healthcare access, fairness and greater overall public awareness.
The law requires that you respond to and fulfill this request "as promptly as possible." If you expect a significant delay in responding to and fulfilling this request, please contact me by phone or via email with information about when I might expect copies or the ability to inspect the requested records.
If you deny any or all of this request, please cite each specific exemption you feel justifies the refusal to release the information and kindly notify me of the appeal procedures available to me under the law.

The Board of Medicine

three professions who self-report whether or not they participate in
Medicaid. Those professions are medical doctors, doctors of osteopathic
medicine, and doctors of podiatric medicine. This information is in the
public domain under Doctors' Profiles <http://www.vahealthprovider.com/>.
Note the entire database for the Doctors' Profiles can be downloaded
<http://www.dhp.virginia.gov/downloads/profiledata.asp>. The Board of
Dentistry has one profession who self-reports participation in Medicaid;
they are dentists who are oral and maxillofacial surgeons. This information
is in the public domain at

No document exists for the informational items on all board licensees you
request other than Doctors' Profiles and License Lookup
<http://www.vahealthprovider.com/>, the static link by which you can verify
the status of a license. You can contact Virginia Interactive
<https://vi.virginiainteractive.org/vi/about.shtml>, a fee for services
entity, to request information on the Commonwealth’s 400,000 health
regulatory board licensees.

Virginia’s health regulatory boards, including the Boards of Medicine and
Dentistry, do not require submission of NPI numbers, and therefore have
nothing responsive to that part of your request. The Department of Health
Professions does not regulate Emergency Medical Personnel.
