Pursuant to the Virginia Freedom of Information Act, I hereby request the following records:
- Any memos, documents or reports related to the Annual Virginia Immunization Survey written or published between Jan. 1, 2017 and the time this request is processed. This would not include any reports already published on the VDH website.
The requested documents will be made available to the general public, and this request is not being made for commercial purposes.
In the event that there are fees, I would be grateful if you would inform me of the total charges in advance of fulfilling my request. I would prefer the request filled electronically, by e-mail attachment if available or CD-ROM if not.
Thank you in advance for your anticipated cooperation in this matter. I look forward to receiving your response to this request within 5 business days, as the statute requires.

Thank you for speaking with Marshall Vogt and myself this afternoon
clarifying your FOIA request to the Virginia Department of Health dated
March 8, 2019. We appreciate your interest in Virginia's immunization
coverage rates and discussing with you the various surveys we use to
analyze trends over time. As we discussed, Marshall and I were able to
describe how the Virginia Annual Immunization Survey (VAIS) is conducted
including its comparison to the School Immunization Survey and answered
your questions. We agreed you did not need more information from the
Virginia Department of Health on this request at this time however you
would explore our website in more detail to see if you had any other
questions. We're happy to discuss any questions or concerns you have in
