The data record layout, data frame, or any equivalent data framework used by this agency to document maternal deaths. Materials responsive to this portion of this request should include reference to the data elements collected by this department and should not include any personal information. Please feel free to redact any personally-identifying information.
If any clarification is necessary, please feel free to call the MuckRock Office at 617-299-1832.
The requested documents will be made available to the general public, and this request is not being made for commercial purposes.
In the event that there are fees, I would be grateful if you would inform me of the total charges in advance of fulfilling my request. I would prefer the request filled electronically, by e-mail attachment if available or CD-ROM if not.

Attached is a file with the death certificate data elements – page 1 is the format prior to July 1, 2008 and page 2 is the format from July 1, 2008 to current. This is the data framework for Vermont deaths.
