Pursuant to the Vermont public records law (1 V.S.A. §§ 315-320), I hereby request the following records:

All EMT visit logs to state correctional facilities between September 2014 and September 2015. The facilities are in Newport, Swanton, South Burlington, St. Johnsbury, Rutland, Windsor and Springfield

The requested documents will be made available to the general public, and this request is not being made for commercial purposes.

In the event that fees cannot be waived, I would be grateful if you would inform me of the total charges in advance of fulfilling my request. I would prefer the request filled electronically, by e-mail attachment if available or CD-ROM if not.

Thank you in advance for your anticipated cooperation in this matter. I look forward to receiving your response to this request within 5 business days, as the statute requires.

I have received your request but require you to clarify what records you are looking for prior to my initiating a search.

Can you please clarify what is meant by “EMT” and “Visit Logs”?
