To Whom It May Concern:
This is a request under the Freedom of Information Act. I hereby request the following records:
A complete copy of the airman certification records—including aeromedical and disapproval applications—of a deceased individual William "Biff" Forrest Hamman (Born: 25 July 1945 | Deceased: 09 January 2014 | Please see obituary published by the Blair-Stubbs Funeral Home: http://www.blair-stubbs.com/book-of-memories/1763867/Hamman-William/obituary.php). Furthermore, the following items are also requested: Flight records and logs for business and government aircraft, including helicopters, affiliated with Hamman; the sale history of aircraft, including helicopters, affiliated with Hamman; and, if extant, any documents regarding accidents, incidents, and enforcement actions, as stipulated in FAA's FOIA compliance rules, FAA Order 1270.1.
The requested documents will be made available to the general public, and this request is not being made for commercial purposes. In order to help to determine my status for purposes of determining the applicability of any fees, you should know that I am a representative of the news media, and this request is made as part of news gathering (i.e. again, not for a commercial use).
I am willing to pay fees for this request up to a maximum of $100.00 USD. In the event that there are fees, I would be grateful if you would inform me of the total charges in advance of fulfilling my request. I would prefer the request filled electronically, by e-mail attachment if available or CD-ROM if not. I also include a telephone number (917.859.1266) at which I can be contacted during the hours of 9:00 am to 9:00pm, if necessary, to discuss any aspect of my request.
Thank you in advance for your anticipated cooperation in this matter. I look forward to receiving your response to this request within 20 business days, as the statute requires.
In order to process the flight and incident data portion we would need specific registration or “N” numbers to perform the search for records. If you could respond providing the N numbers you are referencing in your request, we will be able to proceed in processing your request. Thank you.
