From: J.K. Trotter
To Whom It May Concern:
This is a request under the Freedom of Information Act. I hereby request the following records:
A copy of the United States Army’s Personnel Files on Thomas Bryant Cotton (born May 13, 1977), including performance reports, job assessments, disciplinary records, training received, discharge papers, and any other releasable records.
The public interest in Thomas Cotton, who is a U.S. House Representative and candidate for U.S. Senate in Arkansas, should favor disclosure of many of these records.
I also request expedited processing of this request, due to the enormous public interest in Cotton's career, and the fact that he is actively campaigning for public office based in large part on his military background. The date of the election—November 4, 2014—makes the timely release of the requested information particularly necessary and pressing.
I also request that, if appropriate, fees be waived as I believe this request is in the public interest. The requested documents will be made available to the general public free of charge as part of the public information service at MuckRock.com, processed by a representative of the news media/press and is made in the process of news gathering and not for commercial usage.
In the event that fees cannot be waived, I would be grateful if you would inform me of the total charges in advance of fulfilling my request. I would prefer the request filled electronically, by e-mail attachment if available or CD-ROM if not.
Thank you in advance for your anticipated cooperation in this matter. I look forward to receiving your response to this request within 20 business days, as the statute requires.
Sincerely,
J.K. Trotter

A letter received from your office on January 12, 2015 indicated that the NA Form 13164 would be enclosed, but it appears that it was not. Could you please provide a copy of this form or provide instructions on how to locate it online?
Thank you very much.
