To Whom It May Concern:

This is a request under the Freedom of Information Act. I hereby request the following records:

The Official Military Personnel File (OMPF) including all Fitness Report and Counseling forms (NAVPERS Form 1610/2), the most recent Cumulative Record Of Training (NAVPERS Form 5300/16), the Background Summary (NAVPERS Form 1640/32), Officer Personnel Action Requests (NAVPERS Form 1301/85) and Form DD-214, for Stephen Kevin Bannon, born November 27, 1953.

Bannon is chief strategist and senior counselor to President Donald J. Trump, and has identified himself as U.S. Navy veteran. Because Bannon is a senior member of the White House staff of the new presidential administration, there is a clear and obvious urgency to inform the public about the veracity of his claims to alleged government activity — in this case, his service in the Navy. Please respond with an estimation of the amount of time it will take to process the request.

The requested documents will be made available to the general public, and this request is not being made for commercial purposes.

In the event that there are fees, I would be grateful if you would inform me of the total charges in advance of fulfilling my request. I would prefer the request filled electronically, by e-mail attachment if available or CD-ROM if not.

Thank you in advance for your anticipated cooperation in this matter. I look forward to receiving your response to this request within 20 business days, as the statute requires.

Sincerely,
J.K. Trotter
