CMS Administrator Seema Verma's emails and phone calls with Arkansas, Indiana, Iowa, Kentucky, Ohio, South Carolina or Virginia state officials regarding 1115 Medicaid waivers.
The requested documents will be made available to the general public, and this request is not being made for commercial purposes.
In the event that there are fees, I would be grateful if you would inform me of the total charges in advance of fulfilling my request. I would prefer the request filled electronically, by e-mail attachment if available or flash drive if not.

Any emails or phone call transcripts/notes between March 14, 2017, and present day containing the words "Medicaid" and "1115" and "waiver" between CMS Administrator Seema Verma and:
-- Kentucky Governor Matt Bevin or Kentucky Cabinet for Health and Family Services Acting Secretary Scott Brinkman or former CHFS Secretary Vickie Yates Brown Glisson or Medicaid Director Stephen Miller.
-- Iowa Governor Kim Reynolds or Iowa Department of Human Services Director Jerry Foxhoven or Medicaid Director Michael Randol.
-- Indiana Governor Eric Holcomb or Family and Social Services Administration Secretary Jennifer Walthall or Medicaid Director Allison Taylor.
