Pursuant to the Freedom of Information Act, I hereby request the following records:

All financial disclosure forms for Rosemary Williams, former VA Senior Adviser for Strategic Communications (2009-2011) and VA Assistant Secretary of Public and Intergovernmental Affairs (2016-2017). These forms shall include the Executive Branch Personnel Public Financial Disclosure Report (OGE Form 278e), the supplemental Periodic Transaction Report (OGE Form 278-T) and VA form VA-5655.

The requested documents will be made available to the general public, and this request is not being made for commercial purposes.

In the event that there are fees, I would be grateful if you would inform me of the total charges in advance of fulfilling my request. I would prefer the request filled electronically, by e-mail attachment if available or CD-ROM if not.
