All records of distributions of hydroxychloroquine sulfate and chloroquine phosphate made from the Strategic National Stockpile, including drug name (e.g., Resochin or chloroquine phosphate), manufacturer (e.g., Bayer, etc.), numbers of tablet doses (i.e., numbers of tablets, or alternatively, total units of issue multiplied by tablets per unit of issue), dosage per tablet (i.e., milligrams), recipient name, address and other contact information of recipient, and date of distribution, from March 28, 2020 to the date of processing of this request.

The requested documents will be made available to the general public, and this request is not being made for commercial purposes.

In the event that there are fees, I would be grateful if you would inform me of the total charges in advance of fulfilling my request. I would prefer the request filled electronically, by e-mail attachment if available or CD-ROM if not.
