All records of clinical experience, anecdote, and other unpublished evidence that informed the decision to include use of chloroquine and hydroxychloroquine against COVID-19, as described by the U.S. Centers for Disease Control and Prevention in its document titled "Information for Clinicians on Therapeutic Options for COVID-19 Patients", published online March 21, 2020 at https://www.cdc.gov/coronavirus/2019-ncov/hcp/therapeutic-options.html, to include records related to the specific dosing schedules that "some U.S. clinicians have reported anecdotally" cited in the document.

The requested documents will be made available to the general public, and this request is not being made for commercial purposes.

In the event that there are fees, I would be grateful if you would inform me of the total charges in advance of fulfilling my request. I would prefer the request filled electronically, by e-mail attachment if available or CD-ROM if not.
