To Whom It May Concern:

Pursuant to the Freedom of Information Act, I hereby request the following records:

I would like my personal record of what shows up when my license plate is scanned- License Plate # 289ve4, by the new automated license plate reader. I respectfully ask for the same record that the Massachusetts State Police (any other police agency) see when my plate is automatically scanned. I have no open investigations against me, so your previous exemption for not giving me my personal information is no longer valid. I have reason to believe that there is incorrect information that was manually entered and information against HIPPA. This has caused me much hardship from employment reasons to personal peace of mind. My ex- lawyer filed a discovery motion for my ALPR data in Westfield District court. This lawyer also ended up dropping me 15 days before my next court date and avoided in every way to answering my question on what happened to the discovery motion for my ALPR. I had to call the clerk magistrate to find out what happened with this discovery motion, which I had to pay an extra $2,500 for, and was told that it was never presented in front of a judge. I have a copy of this motion and an affidavit from my lawyer at the time, if that would help to receive my personal ALPR data, which I believe is being used incorrectly. I would like all the people who have accessed it and added manual entries, everything. You can email me this information to my private email. Thank you.

The requested documents will be made available to the general public, and this request is not being made for commercial purposes.

In the event that there are fees, I would be grateful if you would inform me of the total charges in advance of fulfilling my request. I would prefer the request filled electronically, by e-mail attachment if available or CD-ROM if not.

Thank you in advance for your anticipated cooperation in this matter. I look forward to receiving your response to this request within 20 business days, as the statute requires.

Sincerely,

mikayla mucha
