Pursuant to the Freedom of Information Act, I hereby request the following records:

1) All inpatient hospitalization, amputation, and loss of eye data reported to OSHA since Jan. 1, 2013 for establishments in the following industries (followed by NAICS code): fishing (11411); finfish fishing (114111); shellfish fishing (114112); other marine fishing (114119); deep sea, coastal, great lakes transport (48311); deep sea freight transport (483111); deep sea passenger transport (483113).

Title 29, Subtitle B, Chapter XVII, Section 1904.39 of the Code of Federal Regulations states that within 24 hours of the inpatient hospitalization, amputation of body part, or loss of eye of an employee the establishment must report certain data about the incident to OSHA. That data includes: establishment name, location of incident, time of incident, type of reportable event, number of employees involved, name of employees involved, contact person, and a brief description of the incident. Please note that I am requesting data in all the listed fields with the exception of employee names and contact persons.

I ask that all fees be waived as I am a journalist and I intend to use the requested records to publish articles in the public interest and not for any commercial purpose.
