Mike Krzyzewski Military Record

This is a request under the Freedom of Information Act. I hereby request the following records:

The Official Military Personnel File (OMPF) including all Fitness Report and Counseling forms (NAVPERS Form 1610/2), the most recent Cumulative Record Of Training (NAVPERS Form 5300/16), the Background Summary (NAVPERS Form 1640/32), Officer Personnel Action Requests (NAVPERS Form 1301/85) and Form DD-214, for Mike Krzyzewski (DOB: 2/13/1947). He attended West Point from 1965 to 1969, was enlisted in the army from 1969-1974, and had a final rank of captain.

Mike Krzyzewski is a famous public figure. He was the basketball coach at West Point, and as the basketball coach at Duke has won multiple national championships, as well as two gold medals with the USA basketball team.

I also request that, if appropriate, fees be waived as I believe this request is in the public interest. The requested documents will be made available to the general public free of charge as part of the public information service at MuckRock.com, processed by a representative of the news media/press and is made in the process of news gathering and not for commercial usage.

In the event that fees cannot be waived, I would be grateful if you would inform me of the total charges in advance of fulfilling my request. I would prefer the request filled electronically, by e-mail attachment if available or CD-ROM if not.
