Pursuant to the Freedom of Information Act, I hereby request the following records:
Any and all hiring and personnel records for Dr. Leonard Paulozzi, for any paid, elected, appointed or volunteer position he has held in your agency. Paulozzi was employed by CDC for about 25 years, before retiring in 2015. I believe he was most recently a medical epidemiologist in the National Center for Injury Prevention and Control.
These records should include, but are not limited to:
• Job/Volunteer position applications
• Financial and conflict of interest disclosures
• Resumes and CVs
• Letters concerning offers, hire, promotion, disciplinary proceedings and/or dismissals
• Documentation of commendations, recommendations and rewards
• Training records
• Contracts for services
• Salary and bonus records
• Disciplinary actions and/or investigations and/or decisions
• Letters of resignation
The requested documents will be made available to the general public, and this request is not being made for commercial purposes.
In the event that there are fees, I would be grateful if you would inform me of the total charges in advance of fulfilling my request. I would prefer the request filled electronically, by e-mail attachment if available or thumb drive if not. Paper records are my third choice. I do not have the capacity to view CDs or DVDs.
