This is a request under the Freedom of Information Act. I hereby request the following records:
The Official Military Personnel File (OMPF) including all Fitness Report and Counseling forms (NAVPERS Form 1610/2), the most recent Cumulative Record Of Training (NAVPERS Form 5300/16), the Background Summary (NAVPERS Form 1640/32), Officer Personnel Action Requests (NAVPERS Form 1301/85) and Form DD-214, for J.M. Porup.
I, J.M. Porup, held the rank of Midshipman, Fourth Class, U.S. Navy Reserve from 1992-1993. Please include complete military records pertaining to my time in the Navy.
The requested documents will be made available to the general public, and this request is not being made for commercial purposes.

We checked our personnel records and were unable to locate a response record at this command. Depending of the route you took, you would need to contact the Naval Education and Training Command (NETC) if you were in ROTC, or you would need to contact the Naval Academy if you attended there.
