The Official Military Personnel File (OMPF) for JERRY EDWARD MANNING including Form DD-214, all Fitness Report and Counseling forms NAVPERS Form 1610/2, NAVPERS form 1610/1, Enlisted performance evaluation reports NAVPERS Form 1616-1, the Background Summary (NAVPERS Form 1640/32), Cumulative Record of Training (NAVPERS Form 5300/16), history of assignments NAVPERS 1070/605, Navy Training History NAVPERS form 601-4, all military decoration and full award citations, all military education/training courses, military service number.
Specifically request copy of history of assignments NAVPERS form 1070/605, copy of all personal commendation medal citations and all unit commendation medal citation with or without redactions as determined by FOIA officer.

JERRY EDWARD MANNING claims retired navy chief petty officer. Graduate of basic underwater demolition training BUD/S. Approximate service dates 1978 to 1999 active duty. Claimed numerous awards including joint service commendation medal, meritorious service medal, Combat Action Ribbon, Armed Forces Expeditionary Medal, navy achievement medal, Overseas service ribbon and other decorations. birth date unknown, approximately 1952-1957 . This request is submitted to corroborate claim to be a U.S. military veteran. NPRC search of records 1956 to 1994 unable to locate the OMPF file, advised to contact Navy Personnel Command for more information such as military service number, NPRC# or last 4 partial social security number.

I also request that, if appropriate, fees be waived as I believe this request is in the public interest.
