Pursuant to the Freedom of Information Act, I hereby request the following records:

The Official Military Personnel File (OMPF) concerning Jay L. Johnson including Form DD-214, all Fitness Report and Counseling forms NAVPERS Form 1610/2, NAVPERS form 1610/1, all performance evaluation reports , NAVPERS Form 1611, administrative remarks, Background Summary (NAVPERS Form 1640/32), Cumulative Record of Training (NAVPERS Form 5300/16), history of assignments NAVPERS 1070/605, final active duty portrait, Navy Training History NAVPERS form 601-4, all military decoration and full award citations, all military education/training courses, military service number.
Jay L. Johnson served in the United States Navy as a commissioned officer from approximately 1968–2000 active duty. birth date June 5, 1946 Great Falls, Montana. Pilot of F-8J Crusader and F-14 Tomcat. Participated in Vietnam War. Served as Commander, USS Theodore Roosevelt Battle Group and later Chief of Naval Operations, Admiral four-star commissioned naval flag officer. claims awarded many decorations including Navy Distinguished Service Medal , Defense Superior Service Medal, Legion of Merit, meritorious service medal, Air Medal, Navy Expeditionary Medal , Vietnam Campaign Medal, Navy Unit Commendation, Vietnam Service medal, Southwest Asia Service Medal , NATO Medal and etc .

Specifically request copy of history of assignments NAVPERS form 1070/605 and copy of all personal commendation medal citations and unit commendation medal citations with redactions if necessary. Request release of full evaluation reports.
Mr Jay L. Johnson was Chief Executive Officer of General Dynamics and a public figure. His background and qualifications are a matter of public interest. General Dynamics Corporation is an American defense multinational corporation and fifth-largest defense contractor from United States Government.
I am general public and thus do not know the military service number. I also request that, if appropriate, fees be waived as I believe this request is in the public interest .
