This is a request under the Freedom of Information Act. I hereby request the following records:
The following documents related to HMM 364 Squadron at Camp Pendleton from August 2004 through September 2008 (see http://www.med.navy.mil/sites/cpen/services/Pages/IH.aspx):
-Baseline and periodic industrial-hygiene surveys;
-Industrial hygiene technical consultations;
-Special surveys and investigations of employee health complaints;
-Personal and area sampling for vapors, mists, dusts, and fumes;
-Special surveys conducted for indoor-air-quality issues; and
-Outdoor air monitoring tests related to fumes from aviation work, including on helicopters, planes and jets.
The requested documents will be made available to the general public, and this request is not being made for commercial purposes.
In the event that there are fees, I would be grateful if you would inform me of the total charges in advance of fulfilling my request. I would prefer the request filled electronically, by e-mail attachment if available or CD-ROM if not.
