The Official Military Personnel File (OMPF) for Frank Anthony Phillips including Form DD-214, all Fitness Report and Counseling forms NAVPERS Form 1610/2, NAVPERS forms 1610/1, administrative remarks, Cumulative Record of Training , history of assignments NAVPERS 1070/605, all award citations, training courses.
Frank A Phillips served in the United States Navy as enlisted servicemember, approximate service dates 1972 to 1992. Date of birth approximately 1952 to 1960.
Claimed numerous awards including joint service achievement medal, Navy and Marine Corps Commendation Medal, meritorious service medal, combat action ribbon, Navy good conduct medal, Armed Forces Expeditionary Medal, Naval Parachutist badge, Navy Expert Rifleman Medal and other decorations.
