The Official Military Personnel File (OMPF) for WILLIAM M EAGAN including Form DD-214, all Fitness Report and Counseling forms NAVPERS Form 1610/2 - 1610/1, Enlisted performance evaluation reports NAVPERS Form 1616-1, the Background Summary (NAVPERS Form 1640/32), Most recent Cumulative Record of Training (NAVPERS Form 5300/16), history of assignments NAVPERS 1070/605, Navy Training History NAVPERS form 601-4, all military decoration and citations, military training courses.
Specifically request copy of history of assignments NAVPERS form 1070/605, Navy commendation medal citations, meritorious service medal citation and joint service commendation medal citations if possible. Claims numerous Navy decorations/awards and 20+ yrs of military service, rank Chief Warrant Officer, approximate service dates 1975 to 1997, birth date unknown.
