The Official Military Personnel File (OMPF) for THOMAS WILLIAM CASEY including Form DD-214, all Fitness Report and Counseling forms NAVPERS Form 1610/2, NAVPERS form 1610/1, NAVPERS Form 1616-1, the Background Summary (NAVPERS Form 1640/32), Cumulative Record of Training (NAVPERS Form 5300/16), history of assignments NAVPERS 1070/605, Navy Training History NAVPERS form 601-4, administrative remarks, all military decorations and full award citations, military education/training courses, military service number.
Thomas W Casey is a navy cross recipient. Deployed to Panama during Operation JUST CAUSE. Claimed former navy commissioned officer. United States Naval Academy graduate.
Specifically request copy of history of assignments NAVPERS form 1070/605, copy of navy cross citation, joint service commendation medal citations, copy of commendation medal citations, and unit award citations with redactions as determined by subject FOIA officer. Approximate service dates 1987 to 2001 active duty. NPRC search unable to locate the OMPF file.

I also request that, if appropriate, fees be waived as I believe this request is in the public interest . PLEASE NOTE: I am not related to the service member, I am general public and thus do not know the military service number.
The requested documents will be made available to the general public free of charge as part of the public information service at MuckRock.com, processed by a representative of the news media/press.
