To Whom It May Concern:

Pursuant to the Freedom of Information Act, I hereby request the following records:

the Official Military Personnel File (OMPF) including Form DD-214, all Fitness Report and Counseling forms NAVPERS 1610/2 - NAVPERS 1610/1, cumulative record of training NAVPERS Form 5300/16, history of assignments NAVPERS 1070/605, Occupational training history NAVPERS 601-4, all award citations and the Background Summary NAVPERS Form 1640/32 for JOHN GARY GAY. Mr John G Gay claims service in the United States Navy enlisted, retired Master chief petty officer E9, service dates approximately 1977 to 2003, navy occupational rating MASTER CHIEF SIGNALMAN SMCM and claims numerous decorations including silver star medal, combat action ribbon, Navy and Marine Corps commendation medal, armed forces expeditionary medal and other personal and foreign decorations.

I also request that, if appropriate, fees be waived as I believe this request is in the public interest. The requested documents will be made available to the general public free of charge as part of the public information service at MuckRock.com, processed by a representative of the news media/press.

The requested documents will be made available to the general public, and this request is not being made for commercial purposes.

In the event that there are fees, I would be grateful if you would inform me of the total charges in advance of fulfilling my request. I would prefer the request filled electronically, by e-mail attachment if available or CD-ROM if not.

Thank you in advance for your anticipated cooperation in this matter. I look forward to receiving your response to this request within 20 business days, as the statute requires.

I am uncertain as to when you will receive a substantive response to your below request, my hope would be within the next 60 business days. However, we are currently backlogged with complex requests. SEAL personnel records are complex as they require a significant amount of review time.
