Pursuant to the Freedom of Information Act, I hereby request the following records:

The Official Military Personnel File (OMPF) including all Fitness Report and Counseling forms (NAVPERS Form 1610/1 and/or NAVPERS Form 1611-1), the most recent Cumulative Record of Training (NAVPERS Form 5300/16), the Background Summary (NAVPERS Form 1640/32), history of assignments (NAVPERS Form 1070/605) , military awards and full citations, military education/training courses and Form DD-214, for STEPHEN JAMES FITZGERALD .
Mr Stephen James Fitzgerald claims to be a retired commissioned officer of the United States Navy with many years of experience from 1980 – 2004. Claims to be retired Captain O-6.
Please redact any sensitive information protected in accordance with the Privacy Act of 1974.

Mr Fitzgerald has given public speeches discussing military/government related issues & leadership skills and is currently chief executive officer of MAX Digital. He formerly was the Chief Operating Officer of INCISENT Technologies LLC. Mr Fitzgerald has claimed military awards including Legion of Merit, the Bronze Star Medal and the Combat Action Ribbon. I also request that, if appropriate, fees be waived as I believe this request is in the public interest. The requested documents will be made available to the general public, and this request is not being made for commercial purposes.

In the event that there are fees, I would be grateful if you would inform me of the total charges in advance of fulfilling my request. I would prefer the request filled electronically, by e-mail attachment if available or CD-ROM if not.
