Pursuant to the Freedom of Information Act, I hereby request the following records:
The Official Military Personnel File (OMPF) for Richard Donavan Hansen including Form DD-214, all Fitness Report and Counseling forms NAVPERS Form 1610/2, NAVPERS form 1610/1, Enlisted performance evaluation reports NAVPERS Form 1616-1, the Background Summary (NAVPERS Form 1640/32), Cumulative Record of Training (NAVPERS Form 5300/16), history of assignments NAVPERS 1070/605, Navy Training History NAVPERS form 601-4, military awards and full award citations, all military education/training courses, military service number.
Mr Richard D Hansen is a decorated combat veteran with decorations including the meritorious service medal, Combat Action Ribbon, Joint commendation medal, navy achievement medal, Armed Forces Expeditionary Medal and other unit decorations. approximate service dates 1977 to 1999 active duty. This request is submitted to corroborate claims of military service.
NPRC search unable to locate the OMPF file, advised to contact Navy Personnel Command for servicemember discharged after Dec 1994. Specifically request copy of history of assignments NAVPERS form 1070/605, copy of joint service commendation medal citations, achievement medal citations, meritorious commendation medal citation and joint meritorious unit award citation with redactions as determined.
PLEASE NOTE: I am not related to the service member, I am general public and thus do not know the military service number. Mr Hansen worked as senior manager and director with Applied Marine Technologies Incorporated. I also request that, if appropriate, fees be waived as I believe this request is in the public interest . The requested documents will be made available to the general public free of charge as part of the public information service at MuckRock.com, processed by a representative of the news media/press and is made in the process of news gathering.
The requested documents will be made available to the general public, and this request is not being made for commercial purposes.
In the event that there are fees, I would be grateful if you would inform me of the total charges in advance of fulfilling my request. I would prefer the request filled electronically, by e-mail attachment if available or CD-ROM if not.

Performs duties in consonance with the missions of all Naval Special Warfare commands as specified in appropriate Naval War Publications. Performs airborne, diving and demolition operations.
