Pursuant to the Freedom of Information Act, I hereby request the following records:

The Official Military Personnel File (OMPF) for Louis Arthur Kucinski including all Fitness Report and Counseling forms NAVPERS Form 1610/2, all Enlisted performance evaluation reports NAVPERS Form 1616-1, forms NAVPERS Form 1610/1, the Background Summary (NAVPERS Form 1640/32), Navy Training History NAVPERS form 601-4, Most recent Cumulative Record of Training (NAVPERS Form 5300/16), history of assignments NAVPERS form 601-5 and/or NAVPERS form 1070/605, final active duty photograph, all military awards and full citations, military education/training courses and Form DD-214.
Also request Bronze Star Medal and Navy Commendation medal full Citations. Mr Kucinski was a decorated veteran of the Vietnam War with multiple tours of duty.
If necessary please redact any sensitive information protected in accordance with the Privacy Act of 1974.
